HIPAA Confidentiality Agreement

This database includes sensitive and confidential information on both children and support families. Use of the database must be compliant with HIPAA regulations.

You must read and accept the following HIPAA Confidentiality Agreement in order to access the database.

I understand that I may receive confidential information from EveryChild Inc. about children with disabilities living in residential facilities in Texas. Confidential information includes, without limitation, all verbal, written or electronic records. I understand that EveryChild Inc. has permission from each child’s parent or guardian to share information for the purpose of securing an alternate family. I understand that I am receiving this information because I represent an EveryChild partner agency (or other selected provider agency) that recruits and supports alternate family arrangements. I agree to abide by the Health Information Portability and Accountability Act of 1996 (HIPAA), 45 C.F.R. Parts 160 and 164 regarding the use and release of any confidential information that I receive from EveryChild Inc. I will not disclose information that I receive from EveryChild Inc. except as follows: 1) I may share information about a child with a family that is verified by or under contract with my agency in order to determine the family’s interest in caring for the child through a program administered by my agency. I may only share information about a specific child with a family that I know to be qualified to care for the child and that I believe has the right combination of attributes to potentially provide a loving home for the child. 2) I may share information about a child with a family that is verified or under contract with my agency in order to facilitate the exploration of a match between the family and the child, and/or to prepare and support the family to care for the child in their home. 3) I may share information about a child with other staff at my agency in order to facilitate matching children with families verified or under contract with my agency.
